I want a landing page with a lead form submission about catering in the area. Just one really cool SEO optimized page with an easy form to get more information. The target audience is for people in our area looking for catering services.

Elements to include in the landing page

I want a lead generation form that captures basic information about what they are looking for. Dates, food type (drop down list), number of people, budget, if they have a venue, Name, email, phone. We also need a good area to put the information about our company and services. Plus some basic info in the main image slider or whatever area. Testimonials would be nice.
